A: In most states, a minor can enter into a binding contract regarding life insurance above a certain age. North Carolina the minimum age is 15. If the beneficiary is under the age minimum in their state, then the guardian would have to file.
Q: Can you provide me with information on my policy? What is my policy number? When was my policy purchased? (Any general policy information questions)
A: To obtain general information regarding your policy please call the Jefferson Pilot Customer Service Center at 800-487-1485. The Jefferson Pilot Customer Service Representative will only be able to provide you with information pertaining to your policy, if you have question regarding the settlement you must contact the claims administrator.
Q: Is my policy still valid? Does participating in the settlement effect my policy?
A: The settlement does not in any way change Jefferson Pilot's obligation to fulfill the provisions of your policy.
Q: Do I still have to continue making my premium payments?
A. You must continue to make payments on your policy.
Q: I received a packet in the mail recently. What are all the documents I received?
A. CIB Benefit Certificate - The Contributed Insurance Benefit (CIB) is free term insurance. The CIB is offered as part of the General Policy Relief under the settlement. You need take no further action to receive the CIB. You should keep this certificate in a safe place.

Change of Beneficiary Form - The beneficiary under the CIB is the beneficiary named under the policy. The Owner(s) listed on the CIB Certificate may change the designation of the beneficiary for the CIB by completing this form.

Q: I received a packet in the mail, does this affect my policy? Do I still have a valid policy with Jefferson Pilot?
A. The information you received is related to the benefits associated with the class action settlement you were informed of a few months ago. The Insurance Benefit Certificate included in your packet DOES NOT replace any current policy you have with Jefferson Pilot Life Insurance Company. This certificate represent additional (free) coverage being offered under the terms of the settlement.
Q: What is the Contributed Insurance Benefit (CIB)?
A. The Contributed Insurance Benefit is free term insurance. A Contributed Insurance Benefit Certificate ("CIB Certificate") showing the terms of the Contributed Insurance Benefit was included in your packet. Please keep the CIB Certificate in a safe place. You (or the beneficiary) should submit it to Jefferson-Pilot in order to receive payment under the Contributed Insurance Benefit if the Insured dies while the benefit is in effect. The CIB Certificate provides that you will automatically receive the Contributed Insurance Benefit, unless you submit an ADR Claim. Class Members are entitled to receive the Contributed Insurance Benefit as long as the insured on the Policy was alive on December 31, 2002 and is alive on the effective date of the CIB Certificate.

Q: What is the Enhanced Life Policy and the Enhanced Annuity Options?
A. As part of the General Policy Relief under the Settlement you have the option to purchase an Enhanced Life Policy and/or an Enhanced Annuity. If you choose to submit a claim to Alternative Dispute Resolution you may lose your right to purchase the Enhanced Life Policy and/or Enhanced Annuity.
Q: How do I purchase the Enhanced Life Policy/Enhanced Annuity?
A. The packet you received contained a form for you to indicate an interest in purchasing an Enhanced Life Policy/Enhanced Annuity. Upon receipt of this completed form Jefferson-Pilot will provide information on a policy or annuity, instructions on how to apply and will answer any questions you may have.
Q: What happens if I submit a claim to Alternative Dispute Resolution?
A. Once you submit a claim to the Alternative Dispute Resolution you will no longer be eligible for the General Policy Relief, even if you are ultimately awarded no relief in Alternative Dispute Resolution. You should carefully review the Fact Sheet on The Alternative Dispute Resolution Process, to determine if you have the required documentation to prove your claim.
If you elect Alternative Dispute Resolution you must submit a completed claim form along with all required proof to the Settlement Administrator, no later than September 22, 2003.
Once your completed Alternative Dispute Resolution Claim Form has been received, your claim will be reviewed and scored a 1, 2, or 3 by an Alternative Dispute Resolution Claim Reviewer. You will be notified in writing of the decision of the Claim Reviewer.

Sign the ADR Claim Form, which includes a sworn statement, made on your personal knowledge and under penalty of perjury ("ADR Claimant Declaration"), (1) attesting to the truth of the description set forth in the ADR Claim Form, (2) representing that all individuals or entities with an ownership interest have submitted all documents of which they are aware in their possession, custody, or control relating to the ADR Claim or the Policy that is the subject of the ADR Claim, and (3) attesting to the nature, history, and authenticity of all documents and materials submitted with the ADR Claim Form. |

Q: In the Alternative Dispute Resolution process there are three possible scores a claim can receive, what do these scores mean?
A: Score "1" Claims -- A score of "1" means that you did not submit a Sworn Statement of an Unrelated Third Person that supports your claim, you did not submit any written documentation that supports your claim, and your ADR Claim File otherwise contains no such documentation -- and, as such, you will receive nothing.
The Settlement Agreement provides that a Class Member's ADR Claim shall receive a score of "1" if: (a) the ADR Claim File contains no documentary evidence to support his or her claim; and (b) the ADR Claim File has no Sworn Statement of an Unrelated Third Person, which supports his or her claim that the Class Member was provided with an oral and/or written statement or statements that would form the basis of such Class Member's claim that he or she was the subject of a Misrepresentation with respect to the Policy.
A: Score "2" Claims -- A score of "2" means that you did submit with your ADR Claim a Sworn Statement of an Unrelated Third Person that supports your claim, but you did not submit additional documentation that supports your claim and your ADR Claim File otherwise contains no additional documentation, and your actions after the Policy purchase did not render it impossible for the Policy to perform as represented. Score 2 claims receive the relief described below.
A: Score "3" Claims -- A score of "3" means that you did submit written documentation from Jefferson-Pilot or an agent which supports your claim, or your ADR Claim File otherwise contains such documentation, and your actions after the Policy purchase did not render it impossible for the Policy to perform as represented.
